  • Form a team (4-10 people)

  • Pick a captain

  • Click here to complete your registration

  • Every month the captain will re-enter the individual weights

  • Cost $10 per participant. Make check payable to The Wellness Council of Northeast Ohio

  • Send check or money order to Mail Code 0002

  • You will receive monthly health advice and monitoring
